Independent votes in LA not counted due to flawed ballot design

I first heard about this on NPR the night of the elections and also noticed it when I was actually at the polls. The presence of a seemingly odd choice on my ballot asking for a selection of voter type: democrat or decline-to-state. It seems that if an Independent (decline-to-state) voter failed to correctly fill in this bubble, then their vote won't be counted.

To add to the mess, some nonpartisan voters were also apparently incorrectly directed to Republican Party polling booths and marked presidential candidates in that primary. Ridiculous. How hard is it to design a freakin' ballot, anyway? Or train pollworkers to properly advise voters on how to cast their vote?? I'm going to volunteer on election day to try to ensure my neighbors are advised properly on how to make their vote count.
